Please use the link below to do so prior to our first ski day on December 20th. If you have any problem accessing this site, please contact school. We would be happy to set up a laptop for parents to complete these forms.
Info from Jay Peak regarding the waiver: "We don’t need to know any personal info about the parents except emergency contact information and signatures. If the parent doesn’t know the answer to a required field, or doesn’t feel comfortable answering, write n/a or -0- or something in the required field to make the computer program happy in order to proceed and submit the signed waivers. Again, we don’t need to know any heights or weights etc. for the parents." Interested in Foster Care? Join the HOME team! Provide foster care for a local child or youth!
